As the U.S. domestic medical travel industry continues to flourish, the concept of direct contracting with providers is becoming increasingly popular.

This week, I spoke with Bob Cummings, president, National Association of Professional Benefits Administrators (NAPBA), who explains that the concept of direct contracting with Centers of Excellence in the U.S. is primarily driven through a health plan, and for participants who utilize account-based plans, including Flexible Savings Accounts (FSA) or Health Savings Accounts (HSA), they're paying out-of-pocket expenses that aren't covered by the health plan.

In terms of employer direct contracting, employers can establish a special purpose "Medical Travel Health Reimbursement Arrangement (HRA)," and designate tax-free funds that would be available to reimburse employees or pay for a specific menu of eligible medical expenses as defined by the plan. U.S. Domestic Medical Travel (USDMT):  Please describe your members and their business models. 

Bob Cummings (BC):   We currently have about 30 organizations that are members of NAPBA - a grass roots organization for third party administrators (TPAs) of employee benefit programs. NAPBA is focused on promoting compliance standards and best business practices for the TPA industry.

NAPBA members range in size from several larger members that have hundreds of thousands of participants, to smaller members who have roughly 25,000 participants. My company, American Benefits Group, would be considered a mid-sized TPA with about 950 employer clients and 50,000 participants.  

Our access to employers is primarily through the benefits consultant or broker, and a lot of those individuals work in the 100 to 1000 lives market. We also serve a number of large scale clients, some that are well-known companies upwards of 10,000 employees. 

USDMT:  How can providers connect with your TPAs to set up direct contracts with Centers of Excellence (COE), and take advantage of bundled pricing?

BC:  This concept is really driven through the health plan, and for our participants who utilize account-based plans, including Flexible Savings Accounts (FSA) or Health Savings Accounts (HSA), they're paying out-of-pocket expenses that aren't covered by the health plan.

Sometimes the employer is using a Health Reimbursement Arrangement (HRA) to lessen some of the out-of-pocket risk of the deductible.

USDMT: How does employer direct contracting fit into an HRA, HSA or FSA program?

BC:  Employer direct contracting could fit with a Limited Purpose HRA (Health Reimbursement Arrangement). In an HRA the employer provides funds to cover specific medical expenses that are not reimbursed by the health plan. So, for example, an employer could establish a special purpose "Medical Travel HRA" and designate an annual amount of funds that could be available tax free to reimburse employees or pay for a specific menu of eligible medical expenses as defined by the plan.

With an FSA, an employee can contribute $2,550 of their own pre-tax salary a year, or with an HSA, employees can contribute $3,350 for single coverage and $6,750 if they have family coverage in 2016. If they are over age 55 they can contribute an additional $1,000. The HSA allows individuals to save pre-tax, have tax free growth and accumulate funds for future out-of-pocket medical expenses. 

With any health plan design, whether the employee has out-of-pocket cost sharing that is covered by the plan or not, the employee has access to use these funds tax free to pay for any unreimbursed medical, dental or vision expenses.

USDMT:  If employees with an HSA traveled to a COE to receive more affordable, high-quality care, how would employers respond?

BC:  I would think the employer would be very open to promoting this opportunity. If the cost of a procedure is reduced by as much as 50 percent and the employer is paying their medical claims as a self-funded employer it's a win -win. I think they would be very interested in having options available to incent the employee with lower out-of-pocket cost sharing. Any employee responsible for unreimbursed expenses can also be paid tax free from their FSA or HSA account.

USDMT:   How can TPAs present this arrangement to clients?

BC: I think this could be a special purpose HRA -an employer health fund reimbursement arrangement - which is a limited self-insured medical arrangement. Generally, you cannot have a standalone HRA, you have to be covered under a qualified health plan that meets the Affordable Care Act (ACA) requirements. 

For the employers that establish a special purpose HRA (such as a Medical Travel HRA) this could cover these specific COE procedures and expenses. Employers achieve significant claims savings on high cost procedures, and employees are also going to experience less cost sharing than they would under the regular health insurance plan. 

All of my fellow NAPBA members administer account-based plans, and I would be interested in learning more about these kinds of opportunities. I really haven't seen any other account-based TPAs that are involved in COE Medical Travel market to date, but it seems to have great potential.

Right now, I don't market our services directly to employers, but some of our members do. In the case of American Benefits Group, we generally work through the benefit consultant or advisors that control the employer relationship for health and welfare benefits. I would be presenting this solution to the benefit consultants and they in turn would make the recommendation and introduce us to the client.

DiagnosticSite Site Network Certification Awarded to 58 U.S. Imaging Organizations

Healthhelp.com-DiagnosticSite is pleased to recognize 58 organizations for their quality network of diagnostic imaging facilities. To qualify for Site Network Certification, imaging networks must demonstrate that at least 70% of their network is certified through the DiagnosticSite Quality Assessment. Also, DiagnosticSite assesses facilities with interventional radiology and centers offering a low-dose CT lung cancer screening program.

Cross-Border Health Care in California Expands

In my earlier posts on cross-border health care, "Cross-border Workers' Compensation a Reality in California", "NAFTA, Work Comp and Cross-Border Medical Care: A Legal View", "NAFTA, Work Comp and Cross-Border Medical Care: A Legal View: Update", and "Cross-border Health Care and the ACA", I discussed the way some Mexican workers living in Mexico, but working in the U.S. or traveling between the U.S. and Mexico, have been able to get health care on both sides of the border.

An article in Fierce Healthcare.com last month,  says that Scripps Health will help run a hospital in Tijuana, along with Sistemas Medicos Nacionales S.A. de C.V. (SIMNSA).

SIMNSA is the medical insurer in Mexico that the Insurance Company of the West (ICW) contracted with some time ago to treat Mexican workers of ICW's US insureds in the San Diego/Imperial Valley area of CA.
According to the article by Ilene MacDonald, the insurer will design, build and operate the facility, and will seek accreditation from the international arm of the Joint Commission, the Joint Commission International (JCI), and will be an affiliate of the Scripps Health Network.

Those who think that cross-border health care, whether general or work comp-related is not going to happen better think again, because it is, and while this is just now involving the areas along the U.S.-Mexico border, medical travel on this continent is moving forward.

The only thing that is not happening yet is travel further down into Mexico and into the other countries in Central and South America. But that will happen.

I am willing to work with any broker, carrier, or employer interested in saving money on expensive surgeries, and to provide the best care for their injured workers or their client's employees.

I am also willing to work with any health care provider, medical tourism facilitator or facility to help you take advantage of a market segment treating workers injured on the job. Injured workers needing surgery for compensable injuries will need to seek alternatives that provide quality medical care at lower cost to their employers. UnitedHealth to Pay for Kidney Donors' Travel Costs

Fiercehealthcare.com-UnitedHealth will reimburse up to $5,000 in travel and lodging expenses for kidney transplant donors, a move that could encourage more donations and in turn, save insurers money if it reduces how long patients have to wait for a kidney.
